One observation that Navin and I made is that people often times do not know when to ask for help. People and organizations often fall into the "Expert's Fallacy" trap - I know what is the absolute best for my business and my customers, and I really do not need anybody's advice. You have that bright idea/concept, you implement it, you see the initial growth/traction, and then you hit the point where returns on your investments start to diminish/plateau.
